This Notice describes how your medical information may be used and disclosed and how you can get access to this information. Please review it carefully. Norm Wellness LLC complies with the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”).
How we may use and disclose your PHI
- Treatment: To provide, coordinate, or manage your health care (e.g., with clinicians or pharmacies).
- Payment: To obtain payment for services (e.g., billing and claims).
- Health Care Operations: For quality improvement, auditing, and compliance activities.
- As required by law: Public health, law enforcement, or other permitted disclosures.
Your rights
- Access: Request a copy of your medical record.
- Amend: Request corrections to your PHI.
- Accounting: Request a list of certain disclosures.
- Restrictions: Request limits on uses/disclosures (we will consider reasonable requests).
- Confidential communications: Request we contact you at a specific email or address.
- Paper copy: Request a paper copy of this Notice.
Our duties
- Maintain the privacy and security of your PHI.
- Provide you with this Notice and follow it.
- Notify you following a breach of unsecured PHI as required by law.
Uses & disclosures requiring authorization
We will obtain your written authorization before using or disclosing PHI for marketing, sale of PHI, or most uses/disclosures of psychotherapy notes. You may revoke an authorization at any time in writing.

Changes to this Notice
We may change this Notice and make the new Notice effective for PHI we already have and for new information we receive in the future. The effective date will be updated above.
